All forms in the “Attachments” section are required from the prime consultant. In addition, the following forms are required from the prime consultant and each of their subcontractors as part of your RFQ submission: 1. Exhibit 10 H1 - Sample Fixed Fee Cost Proposal; 2. EXHIBIT 10-K Consultant Annual Certification of Indirect Costs & Financial Management System; 3. Debarment Certification Form; 4. Levine Act Disclosure Statement.

How will the participating employers be selected, and how do you envision the consultant coordinating with the employer? i.e. will SACOG facilitate this or will the selected consultant be responsible for garnering participation from employers. - Aug 27 2020 at 5:06 PMSacramento Area Council of Governments
SACOG will be selecting employers through an application and evaluation process. We have an extensive application that will be submitted by the employer and then move through our evaluation form in order to narrow down the final participants. SACOG will facilitate interactions between selected consultant(s) and participating employers.

Yes, with the leading company identified as the prime consultant and the team companies identified as subcontractors/subconsultants.
